Self-management of Lower Back Pain

I want us to understand how we can engage in self-management of lower back pain.  Lower back pain is a common complaint amongst women during pregnancy with about 50% of pregnant experiencing some kind of low back pain either during pregnancies or after. However, lower back pain can be found with people who has undergone surgeries, trauma or old/ elderlies.

However, before we look at how to manage lower back pain, let us understand what is back pain and what are typical reasons / factors that makes people to experience chronic back pain.

In order to present a scientific finding to you, I did some scientific literature search and I stumble on a review paper titled Pregnancy-related low back pain (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3306025/Pregnancy-related low back pain (nih.gov). The research investigated the bodies of works on Pregnancy related low back complaint among pregnant women and its impact on the quality of life.

Low back pain is a common complaint amongst women during pregnancy with about 50% of pregnant experiencing some kind of low back pain either during pregnancies or after. This pain is not new and has been discovered many centuries ago as either Pelvic Girdle pain (PGP) and Lumbar pain (LP). Pregnancy low back pain has been attributed to mechanical, hormonal and other factors.

Mechanical factors are associated with weight gain during pregnancy. As the abdominal sagittal diameter increase, the body gravity center shift anteriorly causing stress on the lower back. Recovery from this require posture change which is quick and not permanent.

Another mechanical factor includes intervertebral discs axial loading that cause compression of the spine. This results in major compression of pregnant women spine with excruciating lower back pain. It takes a longer time to recover from this pain.

Biomechanical factor is another process that impact lower back pain in pregnant women. Muscle fatigue due to abdominal stretching of abdominal muscle to accommodate the enlarging uterus. The fatigue result in extra load on the spine

Some women do experience lower back pain at first trimester of pregnancy. In most occasion, there are no disease or trauma or mechanical changes to cause the pain induction. This led us to another factor which is hormonal changes in the body causing dynamic instability of the pelvis. This theory is still veraciously debated and currently ongoing

Unbelievable, there are large body of work that showed that lower back pain worsens at night when the uterus expands putting pressure on the in the pelvis and lumbar spine. This may be the reasons why most women have back pain at night. The question that I have regarding this factor is why do uterus expands at night? Does it have anything to do with sleeping position or activities of the baby in the stomach at night?

The diagnosis of lower back pain

There are currently very few diagnostic tests and the currently available test are very subjective and the ability of the women to communicate accurately the extent of the pain makes the evaluation more difficult and unreliable

One of ways to test lower back pain is Pregnancy Mobility Index a validated self-report questionnaire. This was developed by Van de Pol et al to quantitatively quantify and assess the ability of doing normal household activities while experiencing back pain.

Women that experience lower back pain in previous pregnancy are very likely to experience back pain. One of the research pointed to a study where 85%  percent of women with back pain in a previous pregnancy developed back pain in a subsequent pregnancy.

On the contrary, women that exercise regularly before pregnancy seems to have reduced risk of lower back pain before and during pregnancy. However, the same it is not true for Pelvic Girdle induced lower back pain

Another theory from Roland questionnaire suggests that the male sex of the fetus may be a predictive factor for back pain during pregnancy. If you are expecting a male child, the study postulated that you have potential to have a lower back pain.

Prognosis for lower back pain

Studies found a direct correlation between progression of pregnancy and  the intensity of the lower back pain. Frankly, I found out that the prognosis for most women with pregnancy related lower back pain is not good. However, the good new here is that postpartum induced lower back pain is a temporary disorder with a good prognosis.

On the other hand, if you gain weight excessively during postpartum, studies suggest that weight retention may trigger lower back pain Hence, desire and actionable steps to actively reduce weight gain is encouraged

Management of Lower back pain

There is great divide between women who seek medical help and those that consider lower back pain as inevitable and normal discomfort during pregnancy
